The purpose of this simple online tool is to determine your general risk and outcome improvement opportunities within your hospital’s CDI department.

Please answer the questions below to get started. Most are multiple choice with required fields to ensure the best results. There are a total of 41 questions regarding your facility with items specific to your CDI department. The duration for completion is 15-20 minutes. Once completed, you will automatically get a calculated score, indicating what ‘zone’ your facility may fall into; RED, YELLOW, or GREEN.

Your organization’s Case Management Department is functioning moderately.  There is room for improvement,  our high level CDI “Health Check” Assessment would be able to provide additional insight as to how to maximize efficiency, improve productivity, effectively meet critical outcome goals, and other opportunities.   In addition, we recommend our subscription services for Chart Audits to ensure best practices and identify ongoing educational opportunities for your staff. Your organization’s CDI Department is high functioning and likely has minimal barriers to best practice. However, we do have packaged services such as our Staffing Ratio Assessment, Learning Needs Assessment, Chart Audits, and Education & Training that may be beneficial to your department.
